About this role

Job Description: Community Health Partners support integrated social care for members with complex health needs (social, behavioral and physical). Responsibilities: Meets with members in the community (home, short-term nursing facility shelter, hospital) as needed, including as an extension of the care team Cares for members, identifying and addressing their barriers to and social influences on optimal health. Work Experience: 2+ years of progressive experience in care coordination, community health, health promotion, or a related field.
